Hello, my name is Miriam Lens. I am the City Clerk and I serve the people of the City of Hayward. I was appointed City Clerk for the City of Hayward in March 2009. I have worked in the Office of the City Clerk for more than ten years taking the lead on such revenue generating projects as the Passport Program in 2007. My office is committed to providing excellent service to our residents and I am proud to have a diverse staff of highly talented and supportive individuals: Yolanda Cruz, Deputy City Clerk, Suzanne Philis and Denise Chan, both Senior Secretaries. I earned a Masters Degree in Public Administration from California State University East Bay and hold a Certificate of Municipal Clerk from the International Institute of Municipal Clerks. I am also a member of the City Clerks Association of California. Bi-lingual in Spanish, I provide translation services for the City and the community. You can reach me at:

Eligibility
To be eligible to register to vote, you must be:
- A U.S. citizen and a resident of California
- Not in prison or on parole for the conviction of a felony
- At least 18 years of age by Election Day
- Not judged by a court to be mentally incompetent
Voter Registration Deadline
The voter registration deadline is always 15 days before an election. Voters who register after the 29th day before an election will not receive a sample ballot or other election information by mail.

In order to remain eligible to vote, you must re-register whenever you:
- Change your residential address
- Change your name (first, middle or last name)
- Change your political party affiliation
Vote by Mail
Any registered voter may vote by mail. You may apply for a vote by mail ballot with the Alameda County Registrar of Voters. The Alameda County Registrar of Voters must receive your application no later than seven days before election.

2012 – EARLY VOTING
Early voting is now available for the November 6, 2012 General Election.
Voters can pick up and return their Vote by Mail ballot at the Alameda County Registrar of Voters’ (ROV) office located at 1225 Fallon Street, Room G-1, Oakland, CA 94512.
The ROV is open Monday to Friday from 8:30 a.m. to 5:00 p.m. The office is also open the weekend before Election Day, Saturday and Sunday, November 3 and 4, from 9:00 a.m. to 5:00 p.m.

The Office of City Clerk has received a Ballot Box in which Alameda County voters can place their Vote by Mail ballot. The office is located at City Hall, 777 B Street, 4th Floor, Hayward. Office hours are 8:00 a.m. to 5:00 p.m., Monday to Friday.

The City of Hayward has consolidated with the California Statewide Direct Primary Election on Tuesday, June 8, 2010.
Polls will be open on Election Day from 7AM to 8PM.

According to the Elections Code, Section 14000, a voter is allowed to take time off for voting. Please see below.
14000. (a) If a voter does not have sufficient time outside of
working hours to vote at a statewide election, the voter may, without
loss of pay, take off enough working time that, when added to the
voting time available outside of working hours, will enable the voter
to vote.
(b) No more than two hours of the time taken off for voting shall
be without loss of pay. The time off for voting shall be only at the
beginning or end of the regular working shift, whichever allows the
most free time for voting and the least time off from the regular
working shift, unless otherwise mutually agreed.
(c) If the employee on the third working day prior to the day of
election, knows or has reason to believe that time off will be
necessary to be able to vote on election day, the employee shall give
the employer at least two working days' notice that time off for
voting is desired, in accordance with this section.

AGENCY REPORT OF PUBLIC OFFICIAL APPOINTMENTS (FORM 806)
Recently adopted State regulation (Regulation 18705.5) requires agencies to complete the Form 806 – Agency Report of Public Official Appointments – and post the completed form on their websites. The form is used to report additional compensation that officials receive when appointing themselves to positions on committees, boards, or commissions of a public agency, special district, and joint powers agency or authority. The Mayor appoints Councilmembers to the various agencies, except for the Alameda County Mosquito Abatement District. Therefore, we are not required to list all agencies, but we are including them for the benefit of the public.
